Transforming Health Care in Arkansas
Heartland Whole Health Connect, formerly referred to as Arkansas Health Care Transformation Demonstration Project, led by Heartland Whole Health Institute, is a statewide effort focused on strengthening primary care and expanding access to care for communities across Arkansas.
Heartland Whole Health Connect brings together primary care providers to explore more accessible, coordinated, and whole‑person approaches to care delivery. By supporting primary care practices as they expand access, integrate physical and behavioral health, and better connect patients to care when they need it, the project aims to improve health outcomes and experiences for people across the state, particularly in rural and underserved areas.
At its core, Heartland Whole Health Connect reflects a shared belief that strong primary care is essential to healthier communities. By working alongside providers and learning from real-world practice, Heartland Whole Health Institute is helping accelerate a more patient‑centered, value‑based approach to health care in Arkansas.
